Empowering Transitions with CYACC and HANDS in Autism
Navigating the healthcare system can be especially challenging for youth with complex health conditions, including those with autism and related developmental needs. That’s why HANDS in Autism® is excited to spotlight a valuable community partner: the Coordinated Youth and Adult Community Care (CYACC) program at Eskenazi Health.
CYACC is a multidisciplinary, team-based program designed to support youth and young adults ages 11–30 with chronic health conditions or disabilities, including autism, who experience medical, behavioral, and/or social complexity. Whether transitioning from pediatric to adult care or needing coordinated services across medical, mental health, and social systems, CYACC helps fill in the gaps.
What CYACC Offers:
- Personalized Care Coordination: Youth and their families receive support in managing complex care needs, identifying goals, and navigating community services.
- Transitional Planning: Especially critical for young people moving from pediatric to adult healthcare systems.
- Mental Health Integration: Support includes behavioral health screening and access to therapeutic services.
- Multidisciplinary Team Approach: Physicians, social workers, care coordinators, and mental health professionals collaborate to develop a cohesive care plan.
